They assess whether the operation can continue delivering safe, compliant, dependable service after ownership changes. For local and regional operators, valuation is not only a finance exercise. It is a test of operational maturity. Reliable trip fulfillment, documented compliance, disciplined fleet management, and a diversified referral base can materially affect both buyer confidence and transaction structure.

## What an NEMT Business Is Actually Worth

Most NEMT transactions begin with a normalized view of earnings, commonly expressed as EBITDA or seller’s discretionary earnings for smaller owner-operated businesses. The buyer then applies a multiple that reflects the quality, durability, and transferability of those earnings.

### Revenue quality and payer mix

Buyers review where revenue comes from, how it is billed, and how reliably it is collected. Medicaid managed care organizations, state programs, healthcare systems, skilled nursing facilities, brokers, private-pay riders, and recurring community partners each carry different risk and margin profiles.

No single payer mix is automatically superior. Public program work can provide volume and predictability, yet reimbursement pressure, authorization requirements, and administrative complexity can narrow margins.
